Short answer: no, modern tooth extractions don’t hurt — you’ll feel pressure but not pain. The procedure is done under local anesthesia, so the area is fully numb. The discomfort comes after, during healing, and that’s manageable with over-the-counter ibuprofen for most patients.
Here’s exactly what to expect, from someone who does these every week.
During the Extraction Itself
You’ll feel numb. We use local anesthesia to fully numb the tooth, the surrounding gum, and the bone. You may feel pressure as we loosen and remove the tooth, and you’ll hear sounds — cracking, suction — but pain isn’t part of the process.
For anxious patients, we offer oral sedation or nitrous oxide (laughing gas) on top of local anesthesia. The sedation handles anxiety; the local handles physical sensation.
Right After the Extraction
You’ll bite on gauze for 30–60 minutes to control bleeding. You’ll be numb for 2–4 hours. As the numbness wears off, you’ll feel soreness — not sharp pain, but a tender, achy feeling around the site.
Take ibuprofen on schedule for the first day, even if you feel fine. Don’t wait until pain starts.
Days 1–3 (Peak Discomfort)
Most patients feel some discomfort for 2–3 days. It’s manageable with ibuprofen for simple extractions. Surgical extractions and wisdom teeth typically need a day or two of prescribed pain medication.
Apply ice to your cheek for 15 minutes on, 15 minutes off, for the first 6 hours. Eat soft foods. Sleep with your head elevated.
Days 4–7 (Healing)
Most patients are back to normal eating and activities by day 4 or 5. The site is still healing internally for another few weeks, but you won’t feel it.
When to Call Us
Call (817) 275-2229 right away if:
- Severe pain that ibuprofen doesn’t touch (could be dry socket)
- Heavy bleeding that doesn’t stop after biting on gauze for an hour
- Fever, swelling that worsens after day 3, or signs of infection
- Numbness that persists more than 24 hours
The Bottom Line
Tooth extractions are uncomfortable, not painful. The procedure itself is essentially painless thanks to modern anesthesia, and the recovery is manageable for most people.
