Submit your insurance info when you book online or give it to our front desk when you call. We verify your benefits with the carrier before your visit and explain your expected out-of-pocket cost in writing — before any work begins.

What's Typically Covered by PPO Plans?
- Preventive (cleanings, exams, X-rays): usually 100% covered
- Basic (fillings, simple extractions): usually 70–80% covered
- Major (crowns, root canals, implants): usually 50% covered
- Cosmetic (whitening, veneers): rarely covered
Coverage varies by plan. Annual maximums (typically $1,000–$2,000 per person per year) and waiting periods may apply.
